Requirements for Parents

1. Parents must fill out a registration form for each child. To register, please click here.
2. At least one parent must be a Baptized Catholic.
3. Parents and Godparents must take a Baptism preparation class. Baptism preparation classes are held the first Saturday of each month in the Family Center Room 109 from 10 a.m. – 12 p.m. Godparents who live out of town may take a Baptism preparation class at their own parish and can fax a proof of having taken a class to the Office of Faith Formation at: (214)357-7802.
4. Parents do not have to be registered members at St. Monica to have their child baptized here. However, if you would like to register to become a parishioner, please click here.
